Covid details for non-patients of dental practices

We will collect details on behalf of dental practices as they are if required by a government directive.
Your contact number will be encrypted.
As of Oct 2020 in Victoria this is the directive:
"Businesses must keep a record of the first name and phone number, and the date and time at which the person attended the facility. Where the patrons spend most of their time in a single space (i.e. a dining room, or a cinema), a record of the space used should also be kept. There is no need to keep multiple records if patrons move through multiple spaces. Records can be kept electronically or in hard copy.
Records should be kept for 28 days. 
The record keeping requirement applies to all persons attending a facility or venue for longer than 15 minutes, which may include staff, customers, maintenance and delivery workers."
These covid specific details will not be used for any purposes other than as required by the government for contract tracing and will be deleted, where they do not form part of the health record, after 28 days.

This information is for individual practices own person use. We will not disclose your personal information to third parties. However, SmartDentist may provide general consolidated information about the SmartDentist subscriber base.
